Shortage of life saving drugs like Methotrexate in the US has emerged as a national health crisis with many health care professionals and patients expressing their concern about the gravity of the situation. The issue has also garnered the attention of Congress, the White House and the media. The shortage of Methotrexate has put many vulnerable cancer patients at high risk. Methotrexate is a chemotherapeutic drug used for the treatment of pediatric patients suffering from lymphoblastic leukemia (ALL). Many children and adults have been benefited by this drug over the past few years. However, the sudden shortage has put the treatment of these children into a state of uncertainty. Hospitals are responding by forming committees of physicians and administrators for determining the possible alternatives to tackle the issue. In several hospitals, the shortage of Methotrexate has forced some physicians to stop the treatment of cancer patients. In some, they are trying substitute medicines so that the recovery of the patients is not hampered. However, these are short term measures. It is essential that some solid steps are taken by the federal agencies to ensure a continuous supply of Methotrexate.

As the shortages of essential cancer drugs such as Methotrexate grip the US, it is time Congress addresses the issue so that such drug shortages in the future can be avoided. The shortage of Methotrexate seems to have occurred due to a complex interplay of multiple factors. The matter has been simmering since a long time. The problem may have begun when Ben Venue Laboratories Inc. stopped its production at the Bedford, Ohio, manufacturing complex in November due to the safety concerns raised by the FDA. Another factor that led to this state is the inability of the FDA to grant permission to other manufacturers for the production of Methotrexate. Although it received several applications from various drug companies for the manufacture of this medicine, the lack of financial resources in the FDA forced it to delay reviewing of the applications. It could not conduct inspection of the factories of these companies due to the same reason. Even the drug manufacturers producing generic drugs were not willing to manufacture Methotrexate as the user group of this medication does not consist of a large base; hence, the profit obtained from the production of this drug is not lucrative for them.

Keeping in mind these constraints, the FDA has announced that it will take steps to ensure the supply of Methotrexate at the earliest. It has been urging drug manufacturers to provide an indication of an impending shortage in advance so that the federal agencies get enough time to manage the situation. The FDA has given temporary permission for shipments from India for some of the drugs facing a shortage. This is expected to ease the situation. It has already granted approval to a new manufacturer for the production of Methotrexate. Some hospitals have been sent a supply of Methotrexate so that the treatment of pediatric cancer patients can be continued for a few weeks till the next batch of supply is arranged for. Still, the FDA is expected to take firm steps to provide a permanent solution to the patients grappling with the shortage of Methotrexate.
